Spondylolisthesis includes anterolisthesis (anterior displacement of a vertebra over the vertebra inferior to it (or the sacrum).) and retrolisthesis (posterior displacement).[notes 2]

Although classification systems exist,[notes 3] the most important parameter is the percentage of slippage as compared to the depth of the vertebral body.

Anterolisthesis is classified by degree of the slippage, as measured as percentage of the depth of the vertebral body, mainly as follows:
- Grade I: 0-25%
- Grade II: 25- 50%
- Grade III: 50-75%
- Grade IV: 75-100%
- Grade V: greater than 100%
